Field Education Forms

Social work students, both part-time and full-time, entering their first semester of study must submit their field education application to the field education department (international students must also complete the International Field Placement Supplemental Application). Once the application has been submitted, students and their advisers will work toward obtaining a field placement. In the school's efforts to find each student a suitable field placement, field agencies will conduct interviews with students, and those agencies will submit this confirmation form once they have accepted a student into their field placement.

Student Learning Goals Sheets

Beginning with the Spring semester for full-time students, each student must submit a goals sheet for each section (SOWK 7520, 7530 and 7540) of their field education practicum. Beginning with their first summer semester, part-time students also must submit a goals sheet for each section (SOWK 7910-7920 and SOWK 7930-7960) of their field education practicum. Field Evaluation Sheets

As part of their field education placements, both full-time and part-time students are evaluated on their full range of performance expectations. With each field practicum assignment, students are evaluated by their field instructors. The following are evaluation forms given to the field instructors by their students to the end of each semester. Any form not listed below is currently being updated and will be available shortly. Student Evaluation Sheets

As the final part of their field education placements, both full-time and part-time students provide evaluations of their field placement and faculty field adviser. These evaluations give students an opportunity to express anonymously their views of their field placement and the performance of their faculty field adviser. The information will be shared with the faculty field adviser only after the student has graduated, and the field placement evaluation will be used to improvement the program as a whole. Both forms are to†be completed and returned to the Director of Field Education at the end of the final week of classes.
